A motivated and experienced support worker with an empathy for aged people and a willingness to be flexible, is required to provide a range of support services for Home Care Package recipients living in the Gosnells and Armadale areas.
Position Description for Support Workers
(Home Care Package Program)


To assist clients of Astley Care with Level 2 – Level 4 Home Care Packages, to remain living as independently as possible, in their homes in the community, by supporting them with a range of services identified on their individual Support Plans.


The HCP Support Worker is responsible to the HCP Senior Consultant or Consultant.



• To provide support services as per the Support Plan for the client and document completion of task.
To ensure that any changes to the clients condition or behaviours are documented.

Personal Care

• Bathing, showering or sponging and toileting of clients as instructed by the Support Plan. Documentation of clients not wishing to bath.

• Assist clients in need of dressing, grooming and/or assistance with feeding as per Support Plan.

• Report all skin tears, breaks, sores and ulcers to the Supervisor.

Home Care

• All housekeeping services including laundry to be completed as per Support Plan to an acceptable standard of work practice.

• Ensure clients needing help to prepare meals are assisted. Report poor appetites to the Supervisor.

Shopping/Medical appointments

• Accompany clients to medical appointments and/or shopping or carry out shopping on behalf of the client, as directed by the Supervisor.


• Support or administer medication for clients in accordance with their Medication Plan. Document and report any medication incidents (eg client refusing or bad side effects) to the Supervisor.


• Provide any duties outlined in the Support Plan including personal care, mobilisation, meal preparation, accompanying the client to medical appointments or social venues and medication management during periods of respite.

• Liaise with the family carer about specific needs of the client on the respite occasion.

• Report back to the Supervisor any changes identified in the client’s/carers needs.


• Report to the Supervisor any changes observed in the client’s habits/practices or in the environment.


• Attend any meetings or training arranged by the Supervisor.

Occupational Health & Safety

• Ensure that work practices comply with Occupational Health & Safety requirements and report any associated problems encountered in the client’s home or environment to the Supervisor.


• Promote independence, in accordance with the Wellness Philosophy, and dignity of all clients and maintain confidentiality.

• Other duties as required by the HCP Senior Consultant or Consultant.

  • Certificate 111 in Aged Care or extensive experience in a smilar role
